Through My Local Surgery, community pharmacists can enhance their contribution to local healthcare, support the primary care recovery plan, and grow their customer base.

Commercial Information for Pharmacies

Here is how the NHS commercial incentive plan would work for them. 

The payment structure for Pharmacy First consultations consists of several components:

Consultation Fee's

Pharmacies receive £17 per completed clinical pathway consultation that crosses the gateway point. This fee is payable regardless of whether an antimicrobial is supplied.

Pharmacies receive £25 per completed Contraception consultation that crosses the gateway point. This fee is payable regardless of whether an antimicrobial is supplied and note that there are no thresholds on the number that can be completed. 

 £20 per emergency contraception consultation (service expanded from October 2025)

Monthly Fixed Payment

  • £500 monthly payment for pharmacies delivering 20-29 clinical pathway consultations per month
  • £1,000 monthly payment for pharmacies delivering 30 or more clinical pathway consultations per month

Consultation Caps

 he NHS has introduced a banding system to manage service growth. Pharmacies are placed into bands based on their average monthly clinical pathway consultations (June 2025 to August 2025 :

  • Band 1 pharmacies (fewer than 18 monthly consultations): capped at 32 consultations per month
  • Band 2 pharmacies (18-24 monthly consultations): capped at 43 consultations per month
  • Band 3 pharmacies (25-30 monthly consultations): capped at 56 consultations per month
  • Band 4 pharmacies (31-34 monthly consultations): capped at 65 consultations per month
  • Band 5 pharmacies (35-43 monthly consultations): capped at 76 consultations per month
  • Band 6 pharmacies (44 monthly consultations): capped at 125 consultations per month

Pharmacies can deliver consultations above their monthly cap, but the £17 consultation fee will not be paid for consultations exceeding the cap limit. Caps are reviewed and updated monthly based on actual service delivery.

This is an excellent solution for both GP practices (removing unnecessary appointments and costs) and pharmacies (driving patient volumes through video consultations integrated with My Local Surgery). The consultation fees combined with the monthly fixed payments create a sustainable revenue model for participating pharmacies while improving patient access to care.
