You can find information below on how to request changes to your practice online patient form and Connect.
If your practice requires any changes to your online patient form, we will need the following information in order to make the required changes.
NB- Any such changes must be submitted and authorised by a practice Super User.

Clinical or Administrative Enquiry Tile changes
New Tiles & Changes to Existing Tiles
- Tile name
- Confirm either an AI or admin pathway
- If an admin pathway please provide the full text and include any links
- Patient comment box on or off (ie visible for patients to submit an enquiry)
- Which unit (team) do you want to route this into?
- Do you want this mirrored on the Connect platform?
- When do you want this change to become live?
Create a New Unit (team) requests
Please note we cannot rename units. If you wish us to create a new unit, please tell us:
- Unit name
- Do you wish to direct a tile to this new unit? If so, which?
- When do you want this change to become live?
To remove a unit that is no longer in use you can choose to deactivate this or soft delete it.
Deactivate a unit (team)
Please note this will not completely delete the unit and a super user will then need to remove access for colleagues within User Administration in the ProUI. We recommend that Super Users keep access to the inactive unit.
- Which unit do you wish to deactivate?
- Does this unit have any tiles linked to it? If so, which and where should we redirect those tiles to?
Soft delete a unit (team)
Please note this will remove the unit and remove all of the archived cases from the ProUI. We will however securely archive the cases and so you will be able to get them back in an urgent situation.
If you wish to do this, please tell us:
- Once you have 'completed' any new cases within the unit
- The name of the unit you wish to soft delete
- If there are any tiles linked to the unit and which team you wish to redirect to (if this is the case)
- When you are hoping for this change to happen?
Add a pop up
Please note that the pop up can only contain 300 characters and it will not be visible during a snooze event.
- What text do you want in the pop up?
- When do you want this to go live?
- When do you want it to be removed?
- Please state if you require the pop-up to be red, amber or green
Re-route a tile into a new unit (team)
- Which tile do you wish to reroute?
- Which team is this currently routed to?
- Which team do you want to reroute to?
- When do you want this to appear live?
