Through My Local Surgery, community pharmacists can enhance their contribution to local healthcare, support the primary care recovery plan, and grow their customer base.

Commercial Information for Pharmacies

Here is how the NHS commercial incentive plan would work for them. 

The payment structure for Pharmacy First consultations consists of several components:

Consultation Fee's

Pharmacies receive £15 per completed clinical pathway consultation that crosses the gateway point. This fee is payable regardless of whether an antimicrobial is supplied.

Pharmacies receive £18 per completed Contraception consultation that crosses the gateway point. This fee is payable regardless of whether an antimicrobial is supplied and note that there are no thresholds on the number that can be completed. 

Monthly Fixed Payment

A £1,000 monthly payment is provided when meeting minimum consultation thresholds. As of February 2025, the current threshold requirement is 25 consultations per month, which will increase to 30 consultations from March 2025. This solution helps drive patients to the pharmacies. 

Consultation Caps

From October 2024, the NHS introduced a banding system limiting the number of paid consultations:

  • Band 1 pharmacies (fewer than 15 monthly consultations): capped at 130 consultations per month
  • Band 6 pharmacies (34+ monthly consultations): capped at 507 consultations per month

The payment system is structured to encourage consistent service delivery while maintaining cost control through consultation caps and threshold requirements. 

So this is a great solution for both GP’s (removing unnecessary appointments/cost) and Pharmacies by driving patient volumes via video consultations.
